Outcomes Stars in children’s social care settings

We’ve been noticing an increase in enquiries about using the Star in children’s social care, which may reflect upcoming changes in the requirements for outcomes measurement and keywork in this sector.

The Department for Education’s recently consulted on a new children’s social care national framework and indicators dashboard, and we were pleased to contribute to this. We welcomed the move towards more person-centred and collaborative care planning and outcomes measurement – the aims set out in the framework being very well aligned with our own (see our Enabling help report). However, we fed back our hope that the final version will include a broader range of indicators and more practical guidance about how to achieve the aims, for example, collaborative assessments to build strong relationships.

The themes of hearing the voices of people being supported, tailoring support and building trusting relationships were also very prominent in the Department for Education ‘Guide to the Supported Accommodation Regulations including Quality Standards’ for looked after children and care leavers aged 16 and 17, which was published in March. Since all providers now have to register with Ofsted and indicate how they will comply with the regulations, we have been working on a guidance document to explain how the Outcomes Star can help with this and how specific versions map across to the principles and quality standards.

Triangle reflects on attending the Scotland that cares – Scottish Parliamentary Reception event – 7th March 2023

I’m Jim Borland, the Implementation Lead for Triangle in Scotland and it was my privilege to attend the recent Scottish Parliamentary Reception event hosted by Oxfam, promoting a new campaign ‘A Scotland that cares’.

The campaign is focused on why making a commitment to valuing and investing in care within the National Performance Framework is so vital to drive progress towards a Scotland that cares. The campaign has been launched to coincide with the first review of the National Outcomes framework in five years. It sets out the argument that to build a fairer and more resilient country, the Scottish government must set a National Outcome for Scotland to fully value and invest in all forms of care and all those who provide it. Too many carers face deep personal and financial costs, including poverty. With the cost-of-living crisis deepening pressures on those who rely on or provide care are increasing. Only once this standard exists, can we then all work together to create positive change for society.

A New National Outcome on Care

The campaign has created a blueprint for a new National Outcome on Care, but it requires public support and the political will to change the current situation and work towards improving the situation for those who experience and provide care across Scotland.

The event was organised by Oxfam Scotland and sponsored by Karen Adam, MSP for Banffshire and Buchan Coast. There were approximately 60 attendees’ from interested agencies, including staff from some of the 55 organisations that currently support the campaign. These included Carers Scotland, Carers Trust, One Parent Families Scotland and Scottish Care, as well as a variety of MSPs from constituencies across Scotland. I am proud to say that Triangle is also one of the supporting organisations.

Support the campaign

Jamie Livingston, the head of Oxfam Scotland opened the event describing his lived experience of caring for his sister who died after battling cancer.  He highlighted the practical issues facing those requiring care and those providing it, but also the issues for the carers once their caring role ends. He talked about his sister’s determination to be active in improving the situation.  Highlighting her efforts of contacting, all the political leaders in Scotland to raise awareness of the lack of support for all those needing and providing care. He has taken his sisters positive action through to this campaign. Whilst many current Scottish political parties already support future change, there is an opportunity for everyone, especially those who experience care and those who provide it to add their voice to the call by taking action via the campaign website: https://ascotlandthatcares.org.

Karen Adam MSP, then highlighted her own lived experience of being a carer, outlining the need for financial support and appropriate resources being made available to help individuals being cared for or fulfilling the very difficult carer’s role itself. She highlighted the need for the carers voice to be heard, valued and rewarded for the work that they do.

Jamie Livingston provided background information on the campaign, which started in 2021 during the Covid-19 pandemic. At the time, it was clear those being cared for, and their carers were excluded from many aspects of the pandemic recovery plans. This was highlighted by the fact care and carers are invisible in all 11 Scottish National Performance framework outcome indicators.  

Together with several partner agencies, the drive to change started with the intention to ensure a permanence to investment in the caring role.  Together with the development and publication of policy via a new National Outcome on Care. Working in conjunction with the University of the West of Scotland (UWS) the partnership developed a blueprint for a new National Outcome. It is hoped to be the basis for potential care indicators specifically linked to carers and the caring role in Scotland. Jamie also highlighted the upcoming consultation phase of the Scottish Government’s review of National Standards and urged individuals and organisations alike to be involved in this consultation and support change to the National Outcomes.

Seven new national indicators have been identified to measure progress:

  • The quality of life of carers, care workers and those experiencing care;
  • The quality of care for all;
  • The financial wellbeing of carers, care workers and those experiencing care;
  • The voice and influence of carers, care workers and those experiencing care;
  • Access to education and training;
  • The adequacy of funding for care;
  • The job quality of social care and childcare workers

Oxfam Scotland: “We want Scotland to fully value and invest in those experiencing care and all those providing it because… Scotland’s one million unpaid carers are the bedrock of health and social care, without whom the care system would collapse. Despite saving Scotland £10.9 billion each year, too often they experience poverty, loss of employment and ill health simply because they care. This must change!”

Satwat Rahman, CEO of One Parent Families Scotland, outlined the importance of this work, and why her organisation has been involved since its inception. Introducing four panel speakers, all with similar negative lived experience of the current support available. These experiences included:

  • The view that the carers’ role is predominately valued and seen as a priority only by family members, peers and caring services. General opinion was that there was little, or no value put on this role by external services, including education, employment, and financial services.
  • There was an expectation from external agencies that carers should ‘know what to do’, without any support.
  • Very limited support available for young carers, especially in the areas of education and finance – thus the carer’s own life and dreams have to be put on hold and suffer whilst they fulfil their caring role.
  • An overly complicated benefits system, which was off-putting to a lot of carers.
  • A lack of resources for those requiring care, or respite for the carers themselves. Demonstrating the need for infrastructure improvements in care provision as one size doesn’t fit all.

Consequently, the need for recognition around the importance of the carers role was emphasised.  Increased and appropriate funding for carers and carers services is needed, to assist carers to provide support and ensure the person requiring care is treated appropriately and with dignity.

Karen Hedge, deputy CEO of Scottish Care concluded the presentations highlighting the view that carers were seen as ‘Cinderella’ within Social Care services, and much more is required to support these individuals. She further emphasised the need for proper policy and legislation which in turn would require achievements to be measured to ensure compliance. Only then would Scotland be able to demonstrate we are a nation that cares.

I found all the personal stories and experiences very moving, particularly from the four panel members sharing their ‘lived’ experience.

As an organisation, Triangle is not directly involved in supporting individuals. We develop Outcomes Star’s for organisations who provide support to individuals in a wide range of social provision settings, including carers. The Carers Star was developed in partnership with the Social Enterprise in East Lothian (SEEL), The Carers Trust and funding from the Scottish Government. It provides a robust framework to assist practitioners work together with carers to help optimise the quality of their lives and assist them in their caring role.

We often discuss the importance of enabling and empowering people by being person-centred and strengths-based and this Star will assist in identifying a carers strengths and challenges to identify support needs to improve the carers situation. However, as highlighted by most of the speakers, the current apathy towards the carer’s role and lack of resources and finance available to support this role directly impacts what is achievable by a person or supporting organisation.

The culture around current care services in Scotland requires change if Scotland is to demonstrate that it values and invests in all individuals experiencing or providing care.

The event gave me plenty to think about and I am personally committed along with my colleagues within Triangle to support this campaign and I would urge anyone reading this post to add their voice to the campaign.

Introducing the new and improved Youth Star to practitioners

Triangle published the Youth Star in 2012 as a light touch tool for community-based youth work. Since then, it has been widely used and the youth sector has changed and evolved. Over the years we have received helpful feedback and realised that we needed to review this Star. Along with updating it, another key aim of the new edition was to develop detailed scales and resources to support training, consistency and clarity as the original edition was published without detailed scale descriptions.

What do I need to do?

If you have been provided with the materials for the new Edition, familiarise yourself with the revised scales, detailed descriptions and new resources (particularly the User Guide and flashcards).

Discuss it with colleagues and try it out before using it with the young people that you support.

What has changed in the new Edition?

Most importantly, we have created detailed scale descriptions for the six areas covered by the Youth Star. These are in a new User Guide, along with a ladder graphic of the short scale descriptions.

In addition, we listened to feedback from young people and workers and changed some wording to be more sensitive, trauma-informed and responsive to the issues people faced and created flashcards and new guidance. Specifically:

  • The first stage of the Journey of Change is renamed stuck, to recognise that things may feel or be stuck for many reasons, not because the young person is ‘not interested’
  • The first scale is now Interests and activities (replacing ‘Making a difference’), and is broader, to recognise activities that are positive for young people but not aimed at making a difference to others.
  • Health and well-being replaces ‘Well-being’ and includes physical health and managing mental health as well as emotional health.
  • Education and work now emphasises getting the most out of school, training, apprenticeships, work or college rather than focusing on achievement.

There are other minor tweaks to scales, with some aspects moving from one scale to another to increase clarity and accessibility.

The new Youth Star is a new and improved edition of an Outcomes Star designed to support services working with young people and in the youth work and community sectors. 

1 Why a new Edition?

Triangle is responsible for ensuring that the Outcomes Stars stay relevant and are as good as possible; we keep them under review, seeking and analysing feedback and developing new editions as needed. We published the Youth Star in 2012 and since then have developed many more Stars for different sectors, leading to new learning, including how to make the Stars and guidance even more trauma-informed, strengths based and person-centred.

The 1st Edition of the Youth Star was published without detailed scale descriptions, so a key aim of developing a new edition was to develop detailed scales to support training, consistency and clarity. Further, Triangle has gathered feedback about the Youth Star and also about how the youth sector has changed and evolved, resulting in helpful feedback. This also pointed to a need to review this Star to make it as good a fit as possible for youth work today.

2 What has changed?

We have created detailed scale descriptions for the six areas covered by the Youth Star. These are in a new User Guide, along with a graphic of the short scale descriptions. The User Guide is most relevant as an important new resource for workers, in training and initial familiarisation with the Youth Star and for reference to ensure accurate and consistent completion of the Star. However, it is written in accessible language and can be shared with young people who are interested and able to engage with more detail.

There is more detail in the Development Report but in summary the main changes are:

  • The first stage of the Journey of Change is renamed stuck, to recognise that things may feel or be stuck for a young person at that stage for many reasons, not necessarily because they are ‘not interested’, as the language in the 1st Edition implied. This also makes this wording consistent with other versions of the Outcomes Star
  • The first scale is now Interests and activities (replacing ‘Making a difference’). This area still emphasises helping others, group activities and teamwork but is widened to recognise activities and interests such as sports, arts, music that are positive for young people but can be individual not group activities and not aimed at making a difference to others.
  • Health and well-being replaces ‘Well-being’ as it now covers both a healthy lifestyle and emotional health, including managing physical or mental health conditions. It recognises that those who are younger may not have much control over their diet or other choices and that their family may not support living healthily.
  • Education and work now emphasises getting the most out of school/ work rather than focusing on achievement. It was also widened to include training, apprenticeships or internships and to recognise the role of the school, college or workplace in providing extra support where needed, so it is not just up to the young person. 

There are other minor tweaks to scales, with some aspects moving from one scale to another to increase clarity and accessibility and adding references to social media and staying safe online. In addition, we listened to feedback from young people and workers and changed some wording to be more sensitive, trauma-informed and responsive to the issues people faced and created flashcards and new guidance.

3 Should I move to the new edition?

We believe that the 2nd Edition of the Youth Star is better, and the additional resources will make it a more effective tool for supporting and measuring change with young people, so we recommend you do transition your service to the 2nd Edition. However, the 1st Edition will continue to be available for the foreseeable future, so you have a choice and time to decide.

The 2nd Edition of the Youth Star is available for those with a Star licence to access on the Star Online, so you can download and it and review the new resources for yourself to help make a decision.

4 What should I do to move the to the new, 2nd Edition?

If you already have Star licences, there is no extra cost to using the new edition and no requirement for additional training, though you may want to consider refresher training. Talk to Triangle about your current licencing and how to make the most of the transition to support good use of the Star.

Consider how you will introduce the new edition to practitioners, for example by finding ways to ensure that they familiarise themselves with the User Guide with detailed scales, changes in wording and other new resources, including the flashcards and updated guidance. This can be done through refresher training, supervision sessions, discussions in team meetings and by sharing and promoting the “introducing the new Youth Star 2nd Edition” poster for practitioners, available from the Star Online or Triangle.  

You also need decide when and how to introduce the 2nd Edition. The main options are:

  1. Introduce the 2nd Edition for new clients and keep using the 1st Edition for existing clients who have already completed at least one Star. The advantage of this approach is that clients only have to engage with one version and their change shown from one reading to the next is valid because it is gathered using the same version
  2. Decide on a date from which to introduce the 2nd Edition with all clients, both new and existing. This is more straightforward for the service because than using both versions and makes the move more swiftly to the new edition. However, change measured from one reading to another is not valid and may be misleading if different editions are used with the same person (see data section below). This has implications for both keywork/ action planning with that individual and for the validity of the service’s Star data overall.

5 What about my Star data?

Because some of the Youth Star scales have significant changes and others have minor changes, data gathered using the 1st Edition will not be directly comparable to data gathered using the 2nd Edition. The most marked example is that someone actively and positively pursuing individual interests such as arts or sports, may be at 1 in the first scale of the 1st Edition (Making a difference) but could be at 5 in the 2nd Edition (Interests and activities). The other differences are summarised above and set out in more detail in the development report.

There are three main options for how to approach data and reporting when making the transition to the second edition of the Youth Star, as set out below

We believe that different options will work for different services. The most appropriate approach for your service will depend on how long you have been using the Youth Star and the amount of data you have and the importance for you of being able to monitor trend over a number of years. Talk to Triangle if you would like advice. However, in general:

  1. We would normally recommend the first option as it’s the most straightforward. Although this is likely to lead to difficulty presenting the data in the year in which you make the change to the 2nd Edition, this disadvantage is short lived, and the issues can be explained to those using the data internally and to funders or commissioners.
  2. The second option is generally only recommended for services with a small data set, particularly those who have started using the Youth Star within the last year, and where it is important to maintain continuity of monitoring over the time of the transition to the 2nd
  3. We strongly advise against combining the datasets from the 1st and 2nd Editions as in option 3, as there are significant changes to the scales that make the transition somewhat like moving to a different version of the Star. However, if you would like to be able to create high level reports across the two editions, we have a briefing called ‘reporting on different versions of the Star’ that would help with this and you can contact Triangle for advice as needed.

Planning Star helps provide sensitive support for young asylum seekers

There was a sharp rise in the number of unaccompanied asylum-seeking children arriving in the UK last year, and children’s services are increasingly having to gear up to support them. It’s not easy – but the new Planning Star for young asylum seekers is ideally suited to the task, explains Triangle implementation manager Marie Buss

In August last year Kent County Council announced that it was no longer able to accept unaccompanied asylum-seeking children into its care – it had simply run out of capacity. The numbers of children, mainly teenagers, arriving on our shores without the care or protection of parents, had escalated, and local authorities in the South East were finding it hard to cope.

The Government’s National Transfer Scheme for unaccompanied asylum-seeking children is now under review to prevent the same thing happening again, and responsibility for these children is likely to be shared more equally around the UK. If that happens, staff who aren’t used to supporting vulnerable young asylum seekers need to be ready to meet their complex needs.

That’s where the Planning Star will really help. Designed in collaboration with the South East Strategic Partnership for Migration and the specialist charity Pathways to Independence, the Planning Star is specifically designed to support these young asylum seekers. It recognises that they arrive in this country without their parents, having undergone terrible experiences, and they can’t be expected to trust people, settle or integrate when they don’t even have a decision that they’re allowed to stay here.

Importantly, it takes into account that these children are traumatised, and it doesn’t push staff into trying to do support plans that may prove unrealistic. It shapes a conversation that covers the right areas and is sensitive to that child’s experience. And it’s very visual, which is crucial for children struggling with learning a new language on top of everything else.

So I think it will be incredibly useful for local authorities, children’s charities and residential services to have the Planning Star in their back pocket. As an implementation manager for Triangle, I’ve done some training with services where they were using the Young Person’s Star (for looked-after children) with everyone, including unaccompanied young asylum-seekers. I’ve shown them the Planning Star and trained them in the difference. And the response has been “thank goodness!”, because the Planning Star recognises that asylum-seeking children are on a totally different journey, and have very little control over their lives.

The Star is very forgiving and it’s very trauma-informed. It accepts that children and young people may be starting with a feeling of “I don’t know anyone, I can’t speak the language, I don’t know anything, I don’t trust any of those adults” – and that’s perfectly normal. Young asylum seekers need time and a lot of help to get to the point where they feel ready to talk and then give things a go themselves. Just getting to “stable”the third stage of the Journey of Change that underlies the Star – is a huge accomplishment for these children.

I love the fact that the Star has one area specifically about immigration with a different Journey of Change that is all about coping with uncertainty and making different plans for different contingencies (known as triple-track planning). And I appreciate how Triangle and its collaborators managed to design it in a way that even if children don’t get the permission to stay in the UK, it’s all about getting them ready for that decision.

It’s about getting children to think, “I need to not get too hung up on the immigration, that’s out of my control. I can only show up when they ask me to show up at the Home Office, but I need to get an education, I need to make friends and I need to think about what my options are in this country, but also be prepared that I may have to return. And I need to think about how I can make the most of this situation while I’m here, and trust the workers to look after me.”

This is something we’re passionate about at Triangle. We’ve done a lot of Stars for children, but I love the fact that the Planning Star is so specialist. All the Stars we design get under the skin of the client group and are trauma-informed, but this one is super trauma-informed. I think people would have forgiven us for not wanting to go down the route of immigration and unaccompanied asylum seeking children – it’s a complex sector, it’s quite legal and confusing – but I like the fact that we did it anyway, for children who will really benefit from it.

One other huge plus – the Planning Star allows people to collate data on asylum-seeking children that captures how they feel when they arrive, what support they accept, what their priorities are, where they’re able to progress, and what areas need more funding. For campaigners and policy-makers that is like gold dust. And if it leads to even better support for unaccompanied asylum-seeking children, that would be truly great.

New Star resource for Young Carers – flashcards for the Carers Star

Sara Burns is one of the founding directors of Triangle and leads on the development of new Outcomes Stars. Here she explains why these new flashcards were needed and how they can help

So far, I have always managed to be in the room when we start developing a new Star. There is something of the texture, feel or nuance of a client group and sector that is communicated by more than words. One day I may have to do that on a screen. While I assume that I’ll find a way, I’d still rather not. I vividly remember standing in workshops many years ago when developing the Carers Star. We collaborated with the Carers Trust and they involved their members – mostly small, local services supporting those caring for a family member or friend. Many in the room were themselves carers or had been in the past, and it was invaluable to hear, imbibe and understand their experience. Although we always carry out a thorough literature review, much of what shapes the Stars, and makes them speak to people, is that deep, ‘bottom up’ listening to workers, service users and others.

People asked for resources tailored to young carers

Ideally, each version of the Star serves a broad sector and range of people or needs and that was our aim with the Carer’s Star. Nevertheless, most people accessing services are older adults, caring for elderly relatives, and the Star is what fits best for them. However, since we published the Carers Star, there have been requests from workers who support young carers for tools tailored to them. In this instance, young carers referring to children and young people caring for a relative at home. This came to a head when I was in Australia in early 2020. I was attending an event for the Australian government Carers Gateway roll-out the Carers Star to all services across Australia. Many services in the sector support young carers too and we agreed to develop flashcards to make the Carers Star friendlier and speak more directly to these service users as well.

Young carers can be both mature and vulnerable

Estimates of the number of young carers in the UK range from 230,000 – 700,000. Those accessing services are a fraction of the total: most are not recognised as needing support and don’t come forward due to stigma, not knowing help is out there or afraid of shaming or being taken away from their family. They are interesting and often impressive. The added responsibility can make young carers mature beyond their years and many express pride at being able to help their family members. Yet as a group they are more vulnerable than others, struggling with education, financially poor and more likely to have health difficulties. Research indicates that they worry a lot – about the health or behaviour of the person they care for, their own well-being, who will do the caring in the future, or about being late and unable to meet the demands of education. They can be isolated, with little in common with their peers.

We created flashcards with words and images relevant for young carers

Flashcards offer an accessible, visual extra resource and are already available for many versions of the Star. To develop flashcards for young carers, we carried out interviews and asked workers to consult with the young carers they supported. We also did a literature review to understand more about the specific needs and priorities for young carers. We produced draft flashcards and asked for collaboration to try these out with young people and gather feedback.

We published the new flashcards for young carers in March 2021, with areas of the Carers Star specifically focused on the aspect most relevant to young people. ‘Work and volunteering’ on the Star is represented by school and education in the flashcards. ‘Finances’ is renamed to ‘money’ and highlights worry about family finances rather than budgeting or banks. ‘Time to yourself’ features football and other leisure activities more relevant to children and young people.

We made changes to the Journey of Change underpinning the Star. Some young carers were alarmed by wording which implied that they needed to be independent of support by the top of the scales and indeed this is not appropriate for children and young people, so we removed it. They also found the end point of ‘as good as it can be’ rather depressing, so that was also changed in the flashcards to ‘things are okay’. The start part point of ‘cause for concern’ also triggered anxiety, so we shortened it to a more neutral ‘concerns’. There were other changes too, but this gives a flavour.

The final version of the flashcards for young carers tested well and can be used flexibly by workers to initiate and support a conversation about the seven Carers Star areas that is much more appropriate for children and young people. For consistency, we have also produced flashcards for the Carers Star for the core audience of adult carers. Both sets of flashcards can be downloaded by clients from their Star Online portal.

The Carers Star is an Outcomes Star for use with people caring for others, it is designed to help organisations and the carers that they support.  It is one of four main Stars which can be used across the adult care sector, other Stars include the Independent Living Star, Life Star or the Older Person’s Star. Organisations that work with young carers may find that My Star or other Stars designed for the family and children sector may be more suitable for working with young people, children and carers under 18. Tyrone’s story: How the Star helped me change my life

Just before lockdown, Tyrone Stanley attended Outcomes Star training to help him in his work to reach violent criminals and gang members. For Tyrone this was coming full circle – because he knows from personal experience just how powerful the Star can be.

But my early life was very different. There was always violence in the home when I was younger, and I started to develop anger issues when I was about 12 or 13. I got sent to prison for the first time in 2011 when I was 16 and got remanded, but didn’t really learn my lesson. I got arrested again and sentenced to two years.

That time I was more productive with my time in prison – I did every single course available to me.

Not a tick box exercise

At the start of a course from the Prince’s Trust, the tutor pulled out the Outcomes Star. She said it was something that was for me, and that was the first thing that made me think ‘oh, OK’. Obviously I was used to doing tick box exercises for different organisations, but the way she approached me for the Outcomes Star was the key element in why it was so beneficial for me.

I was really honest with my answers. I was excited about what the next Outcomes Star would reveal about my progress – it became something to be excited about rather than to be self-critical about. I put an honest reflection of me on paper, and I looked at it and looked at it. Then when I did my next Star I was logging things I was doing, feeling happy about all the good change I’d made.

I got onto an enhanced wing, I even got released on temporary licence. I got out of the bubble I was in – mainly because the Outcomes Star made me see my faults on paper. It gave me a perspective on my actual life – from the outside looking in rather than the false mirage that I’d created.

The key thing in the Outcomes Star that made me think hard was when I realised I didn’t have any long-term friends apart from one or two – they were all connected through badness. I thought about it – like, who are all my friends? Who enjoys my company? I wrote it down on paper and I thought, you know what, I’m not really any good at relationships. Certain things really resonated with me; they made me think I wasn’t the person that I wanted to be when I was a young boy.

It broke me down, man. Peeled off all the layers. Got to the root cause and started again from scratch.

Changing my life

Everything changed – my lifestyle, my circle of friends, the way I spoke. From then on I’ve never stopped working. After various jobs I started with Catch22. My job was to go out and talk to young people about the effects of knife crime and gang culture – and it really helped that I knew exactly what they were going through. Within 18 months I was promoted to my resettlement role.

Honestly, one of the key things in changing the way I thought about myself was being valued – not having people look at me like I was a dreg. I just couldn’t look people in the eye, and I knew no one wanted to look me in the eye. But when I started to do good things, all the blessings started to coming through, non-stop man, non-stop.

And I want to project that to young people. It just goes to show that anyone can change as long as you’ve got the right intervention at the right time.”

Tyrone: How I introduce the Star with service users

OK, so this is an Outcomes Star. Now I know it may seem like a way of recording data for our benefit but I can assure you that this is one of the best proven ways to track your progression. It can also be a great tool to learn things about yourself that you may not have thought about much at all. I have first-hand experience completing an Outcomes Star myself and it was the best start to my journey. By the end I was shocked and amazed at the progression. So be honest, and you will see changes by the end. This is the best way to help me to help you.”
